First Dr recommended AGGA.

Second Dr spoke of Lower jaw advancement surgery and possible upper as well after I have my sleep study. He said my airway is 3 times too small. I asked about MSE but he said my maxilla width is fine its just my teeth that are tipped in.

IMW is 38. Neck posture is bad. Sleep is bad, girlfriend says I'm snoring recently. Daytime fatigue.

Class 2 div 2. 5 mm deep bite. I have TMJ pain on my right joint as well as condyle wear. left joint seems fine. Both click. I had a frenectomy.

I have been developing all the signs of CFD that I've read about on here. I was once square jawed with good posture and slept well. My deep bite has become worse.

Any of you guys have opinions on the attached CBCTs or doctor recommendations?

My Goals are better sleep, more energy, better posture, and hopefully a better face as well.

@engineers

I am seeing a very reputable ENT and will be getting a sleep study done before any decisions are made. I will post those results after they are completed.

@robbie343

Well, orthodontics + DJS + genioplasty would be a good option, definitely

@sergio-oms

Thanks for the response. Based on the CBCT what are your thoughts on lower jaw advancement alone?

@sergio-oms

Basically how do you think my maxilla looks in the cbct scan?

The doctor said it could be a case of just surgery on the mandible.  He seems to think it’s not that retruded, but wants to see my sleep study first and discuss with the surgeon before making any decisions.

Ok .... so what do you think that would happen if you get your lower jaw alone moved forward?

@sergio-oms

Correct my deep bite and get my mandible out of my throat?

Well, maybe, but I doubt your orthodontist will be able to create the necessary overjet for an effective advancement.

 Anyway, I would first start with orthodontics and see. But I’d probably count on having a DJS to get an effective treatment for any breathing issues.
