Hello everyone!
I'm Tomas, 30 year old guy from Austria. Ive been lurking for a while, decided to finally register in the hope to get some advice and feedback on my own case. I'm in the process of surgical treatments, but I'm very much starting to doubt myself and the whole surgical treatment plan (in large part due to this forum).
I don't think I've ever been a mouth breather, but I definitely used to walk around with poor posture (slumped shoulders, forward head, tongue low in the mouth, etc.). I have all the facial features you would expect: lack of zygo width, lack of eye support (basically non-existant cheekbones), narrow jaws, recessed jaws, no chin, etc.
This is all in large part genetic (not a single person in my family has strong bone structure), with genetically small bones. However, the poor posture must have exacerbated the improper alignment of the whole.
Attached are pictures of me, taken around 1 year ago at a surgeons office. Complete lack of pronounced features on a very round and asymmetric head. I feel like a complete mess
I found out about mewing after these pictures were taken, and have immediately started mewing. That was around 6 months ago. I feel my cheekbones have become slightly more pronounced, mostly to buccinator atrophy.
I had SARPE surgery around 3 months ago. Current width around 37mm. I would've liked it to be a few millimeters more, but both surgeon and orthodontist were not comfortable with a further increase (due to narrow lower jaw).
Correct tongue posture / mewing is currently not possible due to the transpalatal distractor against the roof of my mouth..
As you can imagine, I do look a tiny bit better now than I did a year ago in those pictures (no new pics unfortunately, hopefully will have some new ones soon when i go back to the surgeons office).
Next step in the process would be LeFort 1 + BSSO + genioplasty. However, I'm no longer sure of this approach.
Here's a few issues:
- When looking at my pictures, would you say my maxilla is indeed recessed? Or is it just the mandible / chin that is too small and recessed?
- What about the occlusal plane? I'm not sure, but to me it seems to be sloping downwards too much?
- Would bimax-surgery actually be effective in terms of balanced aesthetics?
- I realize some of you are very much not in favour of surgery. Would you say it would be a bad thing in my case? I'm rather skeptical of mewing being able to give me the advancement / rotation / bone growth that I need.
Cheers guys! Hope to become an active member on these forums
Tomas.
