I have no idea what procedures I will require for my problem. I am paying for the surgery completely out of pocket so I am trying to find a decent surgeon however I don't really know what I am looking for to know what constitutes as a good surgeon based off the consultation alone.

I am doing this almost purely for aesthetic/cosmetic reasons not gonna lie, however there are a few minor functional reasons as well. I had four premolars taken out four years ago and to this day I am still doing Invisalign (could not afford DJS at the time, hence why extractions) for my maxillary protrusion and lip incompetence.

My profile/bite has improved however I still would like

* to shorten how long my face is - seems like I have vertical maxillary excess and hyperactive upper lip. I just don't like how long my face looks as it does not look harmonious.
* fix my gummy smile
* fix my lip incompetence - my mouth does not naturally close. Constant practice as well as less teeth have slowly made this better but it's still there. chin mentalis as well. Mouth breather all my life.

I've seen one surgeon who recommended if I wanted the full DJS - 4mm impaction of the upper jaw, some form of lower jaw surgery (he didn't specify what he would do exactly, said he would figure that out during pre surgery testing) and then a genioplasty to fix my lip incompetence (he said something along the lines of my chin being weak hence why naturally my lips do not close)

I am just a little concerned that a genioplasty is going to masculinise my already masculine looking chin :/

Does anyone have any opinions or thoughts? Trying to digest and research as much as possible so I am a little more informed going into my next consult with another surgeon to get their opinion.

Looking at your profile and your jaw positioning, I wouldn't say that your situation is that bad as to merit a surgical approach.

Near where I live there is an orthodontist that provides 'pins' that slowly lift the jawline when used in conjunction with braces over a few years. Maybe see if this approach would suit you a tad more first?

Here's a link to my orthodontist, I am however in Australia, and looking for an orthodontist near you that provides this kind of treatment may be a tad difficult. https://firstbite.com.au/news/fixing-a-gummy-smile-can-be-easier-than-you-think/

Hope this helps!

Not worth surgery in my opinion. Learning lip seal and expanding your palate can do great for widening your face.

Just did - my original account is completely bugged out for some reason.

I have a gummy smile as well (my whole side profile looks similar to you too) and I was a hair's breadth away from doing a Le Fort 1 maxillary impaction for my VME but I heard people say that orthotropics could help reduce gingival display, so that's what I am doing now.

I'll probably have to wait a long time for results, though unlike jaw surgery.

So you mean you have a vertically long nose (sorry didn't see your pic because my browser won't open it)? How to correct it, that's the one million dollar question. I don't think it's really possible but I would follow EddieMoney's advice and do a better lip seal. I mean your surgeon isn't going anywhere, you have time to see if your situation improves without having to spend money.
