Okay, here is another pic of what’s going on with my face lol. I tried to copy the hair and hat, but I am wearing some eyeliner in my waterline in the latest pic as the difference.
Both in cat, at same distance, with the same phone :)
Lately I’ve been trying to mew a lot harder - I think for most of the time I’ve not gotten it “down” with muscle memory or full pushing. But my tongue does feel flatter lately in the last month, and that helps
Great work. I see huge changes. From top to bottom: your eyes look more narrow, your midface looks fuller and shorter, and your lips look wider and more well placed. Also, there is an overall sense of health in the new photo that wasn't there before. Keep it up.
The biggest thing that stands out to me now is the tilt of your midface. It makes it look like your eyes are asymmetrical, but I don't think that they actually are. What I mean is this; if you draw a straight line through the corners of your eyes and a straight line going up the bridge of your nose you would expect them to form a 90 degree angle but in your case it doesn't look that way. Looking at the photo it seems that your midface is tilted off to the right a little bit (which might be your left in real life). I wonder if you are keeping your head at a slight tilt in order to keep your midface vertical - this might explain your back issues if so.
Try this: If you cover up everything under your eyes in the photo with your hand, you can see that your eyes and eyebrows look perfectly symmetrical, it's just that instead of your face coming straight down, it goes off to the side. The good news is that it looks like this has improved from your first picture to your newest one, so it may resolve itself over time as long as you keep doing what you are doing. You can measure the angles yourself to see for sure, I'm just eyeballing it.
Now as for mewing without thinking about it, try focusing more on the neck muscles and not just on the tongue. I figured this out recently and it makes mewing effortless. The best way I can describe it is this: clench down with your jaw and then try to wiggle your ears. If you get it right you should feel a good flex-type feeling right under the back corners of your jaw directly below your ears and the back of your tongue should lift up without any effort. After that just keep your neck muscles flexed and you will stay in proper oral posture without even trying.
Last thing, have you started chewing gum routinely yet? I know you said you have TMJ issues but a lot of the success stories I have found in Mew communities are from people claiming that chewing gum cured their TMJ. It worked for me but my case wasn't so severe. Just something to think about. If you do start chewing gum there's a certain technique I've invented called reverse rotary chewing that can help fix midface tilt.

Good stuff. I second Allixa's excellent dissection. The neck muscles and the tongue should be thought of as one cohesive unit anchored against the pocket created by the palate, the spine and the base of the skull. Significant force is not being generated just upward, but also backward, possibly even more so than forward. The spine and the maxilla are essentially being stretched away from each other, instead of just the maxilla being pushed forward by the tongue. When you "get it", so much starts to make sense in terms of anatomy and movement of the upper end of the spine.
Hi! You know the mention of clenching is good - I’m trying it so I can get it. With my face, the actual skull is asymmetrical from a skull mapping I had done - so I’ll try to keep working on posture. Really good to know you see some changes and I look healthier :) I want to be healthy!
