Yes - you read that right. A bit of expansion for child and improvement in focus! But you may say... that is juat happenstance. You would be wrong. Here's what happened: Schwartz (wish I got Y shape or with two screws in the middle). The split all the way to the incisors had a wide bridge of nose effect. Anyway, had to get that part shaved down a bit... also shaved the palate overall.
Morning alertness improved. Focus improved. Anxiety at night down.
Lost the appliances. Took 2 weeks for fixed replacement.
Mental nosedive. It seemed things were even worse for a bit. Relapse happens so quick.
Got the fixed one in 2 weeks later. Off to good start. Happier with it as it gripped teeth better. Liked the style.. TPA with U in same direction as the teeth arch. Then after a month focus was down! What the?!?! I even got info from teachers. Realized 2 things from meeting 2 individuals... 1... an RPE that angles in toward the nose can mess up nasal cavity area. Straight-out can be unnatural-looking, too, but the TPA was angling more through treatment 2... the TPA waa up so high (against the palate to start)... it was doing a weird thing... creating a gummy smile in back teeth. Oh crap. Super weird... like the pressure wasn't just against sides... but I realized that it touching the palate and preventing it from dropping was bad! Ripped it out. Some gains kept... but what I wanted to have fixed was resolved. Eye circles were dropping lower with TPA against palate... like the problem was that! So now back teeth are coming up (less gummy) and the eye circles are lifted up to better level. Phew! The high arched palate was cutting into nasal space causing the behavior issues to resurface. This is crazy hard and interesting stuff. 🙏🙏🙏🙏🙏 Bless the Lord. He helped me to learn what the problems were by bringing some really intersting case studies into my life. I met 2 women - both middle-aged. 1 had RPE as teen that angled in her face. Part of that were glasses too tight on bridge of nose. Has anxiety, high dopamine, poor job. I like her. She is super smart and knowledgable. The other I met in a Walmart parking lot holding a sign. Her health issues are immense, but her airway is messed up. Had saggital not shaved down. I find this a fascinating case! Hopefully what info I shared helps her. Most orthos don't even do Saggital as much... I swear that God had led me. He even led me to Vivos! That is an awesome story for another day (really long). I had happened to go to an ortho with CBCT. This scan had showed a smaller airway. The pano alone and initial assessment was to wait for treatment. I said no radiation. After hearing that CBCT gives more info on airway... relented to CBCT. Phew! Also,... filled out sleep disordered questionnaire. Sleep always seemed fine. Tested with overnight oxymmetry... it was fine. Dropped below 90 a few times, but not below 88. No polysomnography - just went ahead with ortho anyway. Partly due to concern for nasal allergies. Very, very happy! Dialing it all in. I am pleased to get the aesthetics, allergy prevention, airway heslth perfect. Yes - ADHD no more! ADHD is often just airway. Just a cluster of symptoms that meets DSM threshold.

It is laregely overapplied and misapplied in my observation. In terms of good, better, best it can be good, but to determine what is best for someone takes more than an internet forum. Individualized assessments must be made. There are a growing number of practices who claim to be "airway aware" and just aren't there!!! That is because we are still trying to understand the optimal treatments! Places like orthoscience.com are great, but the top names in ortho fight idealogies like cats and dogs. There are some who push for RPE, others who hate it... and some inbetween. It is super confusing for a mom, which is why I have to rely on my own gut feel, months on endless research, sneaking to get any and all infomation available, countless hours of research and I have enough of a handle to treat my child by getting the appropriate treatment. It is a minefield to navigate! Super scary. I am happy that I have high levels of assessment and understanding that even plenty of the tippy top minds in airway lack! I am grateful for this and humbled to have such a wonderful mind. I am sharing amazingly useful information with the top minds in airway! :-) I will continue to challenge, question & provide useful information!
