I'm 28 years old. About 4 years ago I visited a maxillofacial surgeon due to complaints I had about my facial appearance. He diagnosed vertical growth deficiency of the maxilla combined with sagittal growth deficiency of the mandible and mild cranial scoliosis (asymmetry). I only show 1-2mm of tooth when smiling (and that's when I smile very hard) and my jaws are also canted.
I deferred surgery as I did not trust the surgeons technique, and have since met with 4-5 other high profile surgeons across Europe.
I consulted with Mike Mew for a second opinion and he even suggested I might need a lefort 3, which was shocking to say the least. Needless to say, after he told me that and having seen that Mewing seems to only be applicable for people with long-face patterns, I haven't given much thought to Mewing (though I still try to do it).
Now, several years later, I'm in the position to afford the intervention (Lefort 1 with downgrafting and CW rotation, mandibular osteotomy, genioplasty) all to move the face downwards and forwards.
You can see in the photos (especially the one with the blue background), that my lips look very compressed like I'm pouting, and I look like I have a very deep bite, with the soft tissues making it look like I have a wide face (fat at the sides like a chipmunk). As you can see I'm quite tall but with a very short mid/lower face, which looks quite strange.

I also came across this: /community/community/main-forum/theoretical-question-regarding-mse-fm/ from @melius. they are looking for a way to shorten the face but they mention that Facemask can be used to prevent downwoard growth of the maxilla during MSE treatement. I believe that it might be worth reading it.
Other than that, i can't really help you, i've been lurking on this forum for almost a year now but there are a lot of ppls waaaaaay more knowledgeable than me, especially regarding your case. I've never seen something like your case before on this forum. but i am sure these ppls could help you.

My case is comparatively rare. It's much more common for a maxilla to be too long, as opposed to too short (my case).
I think I should go ahead with the surgery, but the price tag is massive and I'm worried it will cause my nose to look even more upturned at the tip and that I may look strange or uncanny afterwards.
Hey man, that's an interesting case you've got there. I honestly think you can achieve a satisfactory aesthetic result; more so than a case like most of ours in this forum with a maxilla that is too long and down-rotated.

@fknasymmetries Welcome! Your first link was a good reference. The maxilla needs to be lowered...even Dr. Mew said that. This is exactly the result so many here are going for...but as @theratorum found (which I have been saying since Feb...) that Mewing can create problems. Okay.. so... Here’s the issue... your lower jaw came forward a bit. If I had to guess, you pushed the crap out of your roof of your mouth up and forward with pressure. You need to absolutely stop (if you haven’t already). I got reamed in a Case Study saying I don’t know anything because like Mew Mew said to you... Mewing is for adenoid face/craneofacial dystrophy/long face syndrome. These are different names for he thing that Dr. John Mew has. Moving the maxilla down is a good idea...except that your top front teeth have come forward too much. You can see this from the gums piling up. I hope you told Dr. Mew that he’s careless and that his stuff created your problem. You are in luck that I am here to tell you to not chew hard and to let your tongue atrophy and try not to apply pressure from your tongue on the front teeth (top or bottom...). Soft foods only for now. Your tongue is way overpowering your lips. Your gums are bucking in the front. The lips need to be stronger than the tongue. I would slowly try to massage my front lower teeth back (not top). This can be done better with a little widening on the bottom. Dr. Sue Lee can widen on the bottom...the thing is you must first get your lower teeth to sit further back on the mandible... then you can try and get the front teeth together and down. Honestly, braces may be enough just to start. Gum health is super important. Try and not to heavy lifting (or if you do, whatever you do don’t clench and definitely don’t chew gum!!!). Stopping Mewing attempts can be super helpful. The Mewing you are doing is bad for you. After you get your mandible teeth back a bit (widening a bit in the back can help as your arch length is your arch length...). If your tongue weren’t so huge for your mouth (it’s a muscle and muscles grow with use)... then I honestly would consider bicuspid extractions...at least temporarily. That lengthens the face over time. However, with your powerful tongue, you really would just complicate the problem doing that. Has literally no one picked up on your buckling gums? Once you get your tongue from pushing your front teeth out, let it atrophy and get your teeth in a healthier spot... then you can consider options. There are several. But I would wait a few years first.
There are many ways to fix the skeletal issues you have. I am looking at coming up with some myself (to help TMD and high skeletal placement of maxilla... open bites of sorts). Most people start with skeletal and then work on dental. You could do that, too, but in my opinion...it is easier to figure out where everything belongs when the tissues and teeth are properly placed and healthy. If there are bad habits or the tongue is too big from overuse, I think that would be beneficial to resolve first. You can ask these people that...but see what they think. The first thing that needs to happen is stopping the source of the problem. You really need to not be chewing hard stuff!!! It is only driving your molars into the bone so the mandible has to advance for the teeth to touch then the mandible is coming forward... this applies pressure on the front top teeth and your tongue is way too strong. I would try to keep the tongue toward the back of the roof of your palate.
If you want to just be done and over with everything, surgery is the fastest option...but Mewing is honestly has to stop.
For the fastest option, you’re not going to want to hear this... but you’ll need TAD devices as skeletal anchorage all the way around... so like that Won Moon link above (at least it appears he’s using tads...) and extrude all your top teeth. I can’t really see the bottom that well... but to create room you may need some sort of mandible surgery. The reason is because the teeth are too far driven in. You can see it with the gums buckling.
Also, you absolutely must keep your teeth apart. If you want slightly apart - that may be fine, but may not. Do you have TMD issues?

Can you please explain what you mean by the gums 'buckling'? I have never heard this term before from orthos or the literature on the subject..
Also I have already had 4 bicuspid extractions as a teenager, with fixed braces for 2 years afterwards.

Yes, your gums are bowing out near where they touch the teeth. It's got this lip and that is indicative of pushing your teeth up and forward. You're doing excessive chewing - of course your face will be short. WHAT THE CRAP ARE YOU WATCHING ON YOUTUBE? I can't really blame you, because YouTube doesn't care one iota about you or anyone else as a person. Also, the buckling indicates you're pushing your front teeth forward. Of course you may be through your tongue... but it's mostly that damned lower jaw that is coming forward. I don't know what you're doing... or if you even notice this. Your upper-right canine is especially bad. It's like your tooth is getting pushed into the wrong place. I'm so sorry to tell you this... DAMN IT! Can anyone you talk to FUCKING get a clue and PAY ATTENTION!? You obviously need a CBCT (NOT just a panoramic) and I'd consider a water pik (smoothly controlled settings and not just 3) Keep it low and make sure to clean stuff. You need to make sure your teeth & gums are healthy. Don't over-pressure, though... get a good tooth cleaning where they scrape stuff.
