I came across this blog documenting a 30 year old male with class III malocclusion undertaking MSE+FM treatment:
I finished reading through the posts. The cortipuncture procedure his orthodontist used to provoke the suture split is interesting. It might be worth trying in cases that are failing to open. Dr. Moon inserts and removes TADs along the suture like that, but using wide TADs between the roots of the central incisors seems riskier than the skinny drill Dr. Ting advises. I also think cortipuncture is typically done at the time of MSE install rather than waiting until the suture might be on the verge of opening. I didn't read any mention of starting the reverse-pull headgear yet, and the photos don't show the typical J-hooks. I wonder at what stage they plan to start protraction and if they will just hook the elastics around the buccal tubes on the molar bands. I think Dr. Moon often pulls directly from the molar bands to increase the tension on the elastics for adult patients, but it seems like it might be easier to just double up the elastics on the hooks near the canines rather than stretching all the way back to the molars.
