I honestly struggle with the fact I never noticed this before. I’m really sorry that I didn’t share this sooner… It took a while to really know what is important here. As you may know, I’ve been to many different orthodontist for consultation. The airway experts had wanted to slowly expand the arches, thereby dropping the palate. I should’ve connected this sooner, but now that I have I must share. We know that if the palatal vault is too high that it can interfere with the nasal passageways. Typically, this is associated with narrow arches. However, some orthodontists look at the arch distance only. They are not aware of the impact of a high palatal vault. I hadn’t been either. But now I know! A high palatal vault is associated with a few problems. One is that the tongue has nothing to grab onto if it’s too high. In fact, I was talking with my doctor today. He has a patient who would put a silver dollar high up in their palate. This was to treat their OSA. It acted as a tongue depressor of sorts. Unfortunately, complications arose from using that silver dollar and they are hospitalized. It sounded as though it may have come loose from the intended position. So, the real trick is how to lower a high power little vault? And, although I’m not a doctor, I’m going to share this... Any orthodontist who really understood airway that I spoke to did not like rapid palate expansion. I’m joining their camp today. Sure, they’ll use it in extreme situations. Unfortunately, it can end up growing the palate. Instead, a slower action can drop the palate depending. It is relatively easy to widen a palate. Once over-widened, game over. I know of nothing to reign in an oversized palate.
Perhaps braces over RPE? Perhaps DNA over SARPE and DOME and the like?
Good post. I have a high palate and suspect that's why tongue posture has never felt natural to me in the 3+ years of trying to improve my face, also why I've gained no changes. I was looking into the DNA appliance about a month ago but decided against it after seeing a lot of complaints. Pretty sure they were about lack of maxillary movement and/or teeth tipping (which seems like a criticism thrown at a lot of appliances).

My regular doctor's patient! There are tongue depressors and crap out there.. but I don't think they hit the nail on the head. Literally... why would DNA (group case study) help OSA!?!?!? AFTER treatment... with NO DEVICE???? Yes, I saw this case study! The reason why is 2-fold... the widening of the arches helps to widen the airway behind... AND! The DROP of the palate.
How many stories are we going to put up with where a kid gets palatal expansion and shows improvement. What about long-term? Are these kids getting tracked long-term?!?!??!?!? I mean... a Hyrax is literally a tongue depressor. I'm concerned about the new bone growth. Once you overgrow... you can't go back. You literally are game over. I saw a girl overexpanded on maxilla. Literally could not bite with both back molars touching. Scary crap. You can ALWAYS widen....
DOME and all these TAD assisted things.. they may not hold a candle to what's coming next. I suspect what's wrong with everything out there...why it works,... why it doesn't... how to design the optimal expansion (TRUE) for adults. I think we can do this.
