How the extreme change at his age just from these two things?

The after picture of his side profile wasn't taken at the same distance. It's closer, so the lens is going to blow everything up near the center, creating the illusion that his head got wider.
Again, the X-ray "change" shows his entire head rotating uniformly. Proportionally, everything is the same.
The upper palate pictures are taken at completely different angles. Look at the front incisors change angle. The change in angle makes it seem like his palate widened. It did not.
Look at the final icat image. His upper jaw is mostly green. Green means there was no change. The previous comparison of his upper palate shows all of his teeth moving a lot! (seemingly palate expansion), and yet here they are mostly green, meaning they didn't move at all!
Lastly, keep in mind that this is mostly a sales pitch. He's selling this appliance. He's pretty disincentivized from being too critical. But I don't think he's a bad guy. I think he's pretty fooled himself.

You have to perceive it all rotating in 3d space. For the upper palate, imagine the front of his mouth coming down and towards you and the back part moving away from you and swinging up. Watch for the angle of the front teeth, the shadow in his mouth, and the shape and rotation of his molars. You see it in 2d because that's what's been suggested to you. Just gotta break out of that.
